There are a number of reasons why you might get disconnected like that (happens to me too). The worst part of this is the not being able to get back in part. Far as I can tell, it's because the server thinks you're still on. I'll often notice, when I'm disconnected like that, if I check the server I'll see that I'm still logged in. Sometimes it can take quite some time before you actually get logged out.
I actually don't have a fix for this, but if anyone does, I would also be very appreciative.

I get it at times as well when my connection is being slow or the like.
Try Direct Connecting, it helps occasionally ^.-
Other then that.. dunno =/
